Before you travel to the office for a dental appointment, we ask that you:
- Reschedule non-emergency dental treatment if you are feeling ill or have had a fever in the last 24 hours. Even if symptoms develop the day of your appointment. And, If you have tested positive for COVID-19 within the last 10 days even if you are asymptomatic. Please call to let us know if you have had close contact with someone who has been diagnosed with COVID-19 in the last 10 days.
- Complete the “COVID-19 Pre-Appointment Screening” form if you have not done so in the last 14 days and review the “Medical History Form” from your patient portal. (Note: the email address used to register should be the same email address on file with our office)
- Bring layers of clothing to keep you warm during your visit. (Due to PPE requirements, we will keep the temperature in the office cool to ensure the comfort of clinical staff.)
- Let us know if you have had any surgical procedures since your last appointment, so we may obtain appropriate clearance if necessary.
- Contact us if you have had a dental insurance change since your last appointment. It is important we have this information before you get here.
At your appointment, we will:
- Obtain your vitals and review your medical history, including all medications and supplements you are currently taking. If you have a list of medications, please bring this to the office at the time of your visit.
- Request that you review and initial a short questionnaire and sign an informed consent for treatment. We will answer all questions you have before starting dental procedures.
- Ask that you swish for 30 seconds with a mouth rinse before we begin your treatment.
